Memory Assessment - Initial Consult

Service Description

Patient and family attend the Comprehensive Cognitive Assessment with to discover memory concerns, review diagnoses and medications, chronic health conditions, and events that may have contributed to the memory or cognitive condition, current living situation, and other aging concerns. Evidence-based tools, completed with the patient and family members, provide the basis for evaluating the patient's current and future care needs. Results and recommendations are discussed in detail. You will be provided your own copy of the Report to take with you; upon request, the Report is provided to your medical team.
